PRIVACY POLICY OF ALIVIA HOME DELIVERY
Alivia Home Delivery (“ALIVIA-HD”) is a service provided through a mobile application and a website owned and operated by a third-party contractor, not by Alivia Home Delivery itself. Medications purchased through ALIVIA-HD will be dispensed by Alivia Home Delivery, LLC, a duly licensed pharmacy in Puerto Rico (the “Pharmacy”), and delivered by a courier contracted by the pharmacy.
Your access to ALIVIA-HD’s mobile applications and websites is subject to the Terms of Use, Disclaimers, Express Consent, and this Privacy Policy (“Privacy Policy”).
General
We may collect non-personal information and personally identifiable information through ALIVIA-HD, which may be used to provide you with personalized services, content, industry-related information, and for ALIVIA-HD’s general business purposes.
By using ALIVIA-HD, you accept and agree to be bound by the Terms of Use and this Privacy Policy. Specifically, by using ALIVIA-HD, you consent to the collection, use, and disclosure of your non-personal information and personally identifiable information obtained through ALIVIA-HD in accordance with the Terms of Use and this Privacy Policy.
This Privacy Policy may be modified at any time without prior notice and at our sole discretion. Amendments to this Privacy Policy will become effective on the date they are published on the ALIVIA-HD application or website. Therefore, you should review this Privacy Policy periodically.
Your continued access and use of ALIVIA-HD after the effective date of any amendments to this Privacy Policy signify your acceptance of these amendments. If you do not agree to be bound by (or cannot comply with) the terms of this Privacy Policy as amended from time to time, your sole remedy is to stop using ALIVIA-HD. To take advantage of ALIVIA-HD and receive electronic communications from ALIVIA-HD (if applicable), you must be a registered user.

Geolocation Information / Mobile Devices
Certain features and functionalities of ALIVIA-HD depend on your location. To provide these features and functionalities, we may—with your consent—automatically collect geolocation information from your mobile device or cellular provider (collectively, “Geolocation Information”).

Geolocation Information is collected only when the ALIVIA-HD app is running on your mobile device.

You may refuse to allow us to collect your Geolocation Information. However, if you do so, we may not be able to provide certain features or functionalities.

When you turn 18, we may:

Permanently disable the Authorized Person’s account

Confirm whether the Authorized Person may continue acting on your behalf

We are committed to preventing the unintentional collection of Personal Information and Health Information from children under 13 years old.
If you are a parent or legal guardian of a child under 13 years old and believe that they have directly provided Personal Information or Health Information to us, you have the right to review that information or request its removal from our database.

Cookies
When you visit or register with ALIVIA-HD, your computer or device may receive a “cookie” or “GIF file”(collectively, “Cookies”) to assign a unique and anonymous identifier to your browser, computer, and/or device.
A Cookie is a small data string that allows ALIVIA-HD to remember you, similar to a membership ID.
